Statutory Declaration Forms

Members who have 'lost/misplaced' their membership certificate and wish to get replacement are requested to submit a replacement form and statutory declaration form together with replacement certificate fee of RM 10.00 (with proof of police report) or RM 50.00 (without valid reason).
